Skilled Nursing

Skilled Nursing services rendered by licensed Registered Nurses (RNs) or Licensed Practical Nurses (LPNs) who have received specialized training and have experience in providing medical care. After a initial assessment and evaluation is conducted by an RN, follow up visits are completed by LVNs/LPNs on an as-needed basis depending on the physician’s recommended frequency and treatment plan. Each patient’s frequency is determined by their individual medical needs and can be adjusted for flexibility of scheduling.

Physical Therapy
Physical Therapy services are rendered by registered Physical Therapists and Physical Therapy Assistants (PTAs). The goal of Physical Therapy services is to provide patients with the tools and resources needed to regain their independence and increase their quality of life. This may include improving mobility, reducing pain, and increasing strength and endurance, allowing patients to perform daily activities with greater ease and confidence.
Occupational Therapy
Occupational Therapy services are rendered by registered Occupational Therapists and Certified Occupational Therapy Assistants (COTAs). The goal of Occupational Therapy services is to provide patients with the tools and resources needed to improve their ability to perform daily activities and achieve greater independence. This may include improving coordination, increasing strength, and using assistive devices and equipment to make daily activities easier and safer.
Speech Therapy
Speech Therapy services are rendered by registered Speech Therapists. The goal of Speech Therapy services is to provide patients with the tools and resources needed to improve their speech, language, and swallowing abilities, allowing them to communicate more effectively and safely consume food and liquids. This may include improving articulation, increasing vocabulary, and using communication devices and technology to facilitate communication.

Medical Supplies & Durable Medical Equipment
The goal of Medical Supplies and DME services is to provide patients with the necessary supplies and equipment to manage their medical conditions and maintain their independence in the comfort of their own homes. This may include providing wound care supplies to prevent infections, providing mobility aids to help with mobility and daily activities, and providing oxygen equipment to help with breathing difficulties. Home health care companies work closely with patients and their healthcare providers to ensure that they receive the appropriate supplies and equipment, and that the equipment is properly maintained and functioning.
