Our team of skilled professionals includes registered nurses, physical therapists, occupational therapists, speech therapists, medical social workers, and home health aides, all of whom are dedicated to helping patients manage their health conditions and improve their quality of life. We understand that every patient has unique needs, and we work closely with each patient and their family to develop a personalized care plan that meets their individual goals and preferences. Qualifications

To qualify for services, the patient must be under the care of a physician, be homebound, and in need of intermittent skilled care. This may include observation, instruction, or hands-on care. At Ease Home Health Care works on a referral basis. Though referrals typically originate with the physician, they may originate from any local hospital, doctor’s office, and/or skilled nursing or board and care facilities. Referrals may also come from the patient’s family, other interested persons, or home healthcare agencies. Between 24-48 hours after a representative from our home health agency has been alerted to the patient’s needs, the patient will be seen and evaluated so care can begin. Under certain circumstances, special acute care can be rendered the same day.
