At Ease Home Health Care is a licensed home health agency and a covered entity under the Health Insurance Portability and Accountability Act (HIPAA). Protected health information we maintain about our patients for treatment, payment, and health care operations is governed by our Notice of Privacy Practices, not by this Privacy Policy. This Privacy Policy covers information collected through the Site.

Information You Provide to Us
- Contact and care request forms. Your name, email address, phone number, your message, and, on our Get Care form, the name and phone number of your primary care physician.
- Employment applications. Your name, date of birth, gender, the position you are applying for, email address, phone number, street address and city, whether you are eligible to work in the United States, your home health and previous work experience, and your electronic signature and date.
- Referral and patient care forms. Information you or your physician provide on referral paperwork returned to us by fax, mail, email, or in person.
- Direct communications. Information you share when you call, email, text, fax, or message us on social media.
- Anything else you choose to share. This includes health-related details about yourself or a loved one that you volunteer when describing your care needs.
How Care Request Form Information Is Handled
When you submit a care request, the health-related details you provide, including your message and your physician's name and phone number, are delivered to our staff by email and are not stored in the website's database. What the website retains is limited to your contact details, whether you agreed to receive text messages, the exact wording of the consent you were shown, and the date, time and IP address of your submission. We keep that record so we can demonstrate that consent was given.

When you visit the Site, we and our service providers may automatically collect:

Text Messaging (SMS)
Our website forms include a separate, optional checkbox for text message consent. It is never pre-checked, and you can submit any form without checking it. If you check it, you agree to receive text messages from At Ease Home Health Care at the number you provided, about your care request, scheduling, and care coordination. Consent to receive text messages is not a condition of receiving services or of employment. Message frequency varies. Message and data rates may apply. We do not use your mobile number for third-party marketing, and we do not sell, rent, or otherwise disclose it for marketing or promotional purposes.
Mobile information will not be shared with third parties or affiliates for marketing/promotional purposes. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.

Data Security
We use administrative, physical, and technical safeguards designed to protect the information we collect, and we limit what the website stores so that health-related details are not held in the site's database. No website or method of transmission is completely secure, so we cannot guarantee absolute security. Please use the phone number above rather than a web form for anything sensitive.
How Long We Keep Your Information
We keep personal information for as long as needed to fulfill the purpose it was collected for, and afterward as required by state and federal recordkeeping, licensing, and accreditation rules. Text message consent records are kept for as long as we may need to demonstrate that consent was given. Patient records are retained for the period required by California and federal law.
Your Privacy Rights
Depending on where you live, you may have the right to:
- Know what personal information we have collected about you, where we got it, why we collected it, and who we shared it with
- Request a copy of that information
- Request correction of inaccurate information
- Request deletion of your information, subject to legal and medical recordkeeping requirements
- Opt out of the sale or sharing of your personal information. We do not sell personal information, and we do not share it for cross-context behavioral advertising.
- Not be discriminated against for exercising any of these rights
Medical records held by At Ease Home Health Care as a HIPAA covered entity are handled under HIPAA and California medical records law rather than under general consumer privacy law.
