Skilled home Nursing services may include but are not limited to:
- Assessment and monitoring of a patient’s medical condition, including vital signs, management and monitoring of side effects and symptoms, and skilled nursing teachings/education.

- Wound care treatment and monitoring in accordance with physician orders.

Home Physical Therapy Services
Home physical therapy services may include but are not limited to:
- Assessment and evaluation of a patient’s mobility, balance, strength, and function.
- Development of a personalized treatment plan to improve mobility and function.
- Implementation of therapeutic exercises, stretches, and activities to improve mobility, balance, and strength.
- Use of assistive devices and equipment, such as walkers or canes, to promote safe movement.
- Education on disease management and strategies to prevent falls and injuries.
- Coordination of care with other healthcare providers, including physicians, nurses, and other therapists.
Home Occupational Therapy
Occupational Therapy services may include but are not limited to:
- Assessment and evaluation of a patient’s ability to perform daily activities, such as dressing, grooming, and cooking.
- Development of a personalized treatment plan to improve the patient’s ability to perform daily activities.
- Implementation of therapeutic activities and exercises to improve coordination, strength, and fine motor skills.
- Use of assistive devices and equipment, such as splints or adaptive tools, to promote independence and safety.
- Education on disease management and strategies to prevent injury.
- Coordination of care with other healthcare providers, including physicians, nurses, and other therapists.
Home Speech Therapy
Occupational Therapy services may include but are not limited to:
- Assessment and evaluation of a patient’s ability to perform daily activities, such as dressing, grooming, and cooking.
- Development of a personalized treatment plan to improve the patient’s ability to perform daily activities.
- Implementation of therapeutic activities and exercises to improve coordination, strength, and fine motor skills.

Certified Home
Health Aide
Home Health Aide services may include but are not limited to:
- Assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Assistance with Instrumental Activities of Daily Living (IADLs) such as light housekeeping, meal preparation, and transportation.
- Assistance with medication management and reminders.
- Basic health monitoring such as taking vital signs.
- Reporting changes in a client’s condition to the registered nurse or licensed therapist.
- Providing companionship and emotional support.
- Assisting with exercises or therapy programs prescribed by a healthcare provider.
Medical Supplies & Durable Medical Equipment at Home
Medical Supplies and DME services are offered through third party equipment companies, which may offer the following services (but are not limited to):
- Delivery of medical supplies such as wound care dressings, incontinence supplies, and diabetic supplies.
- Rental or purchase of Durable Medical Equipment such as hospital beds, wheelchairs, walkers, and oxygen equipment (overseen by the DME company).
- Set-up and installation of DME equipment in the patient’s home.
- Education and training on the safe and proper use of DME equipment.
- Regular maintenance and repair of DME equipment.
- Coordination of insurance coverage and billing for medical supplies and DME equipment.
Mobile Laboratory Services
Mobile laboratory services that are ordered by the physician are coordinated through a third party contractor and overseen by us.
